Intensive care coordinators (ICC) help children or teens under the age of 21 with serious emotional disturbance. An intensive care coordinator develops a customized and flexible plan of care and makes sure the child or teen gets the right medically necessary services at the right time.

What You Will Be Doing to Make a Difference...

Acts as a liaison with different collaterals, programs and family members to establish and maintain relationships/services that support children and family growth.
Actively assist youth and families to obtain the delivery of available services including medical, educational, social, therapeutic, or other necessary services including formal and natural supports with community-based agencies, services, and organizations.
You will have the opportunity to aid in each familys unique mental health journey wherever it may take them and providing consistent support and guidance.
You will have the ability to develop a broad network of resources and services within the many systems of care that impact the youth and families we serve.
Perform initial assessments and evaluations of new clients and families. Develops, implements, and monitors Individual Care Plans and Transition Plans.
Documents significant client interactions per MassHealth requirements.

Requirements:

Credentials/Qualifications

Masters degree in Human Service field
Bachelors degree in Human Service field and minimum 1 year relevant experience in navigating the child/family serving systems and experience advocating for family members who are involved with behavioral health systems; OR
Bachelors degree in non Human Service field and more than 1 year relevant experience in navigating the child/family serving systems and experience advocating for family members who are involved with behavioral health systems; OR
Associates degree or High School Diploma and minimum of 5 years relevant experience in navigating the child/family serving systems and experience advocating for family members who are involved with behavioral health systems
Experience in navigating the child/family serving systems and experience advocating for family members who are involved with behavioral health systems
Must be certified in MA CANS
Valid MA drivers license with acceptable driving record and current proof of insurance
Acceptable CORI and SORI background record check as required by program
Bi-lingual in Spanish desired
