Schedule:Mon,Tue 8am-8pmWed,Thu 8am-4pmWeekend hours required on rotational basisRate:$75,000/yrSign On Bonus:$5000; $1666.66 upon completion of 30 days, $1666.66 upon completion of 6 months, $1666.67 upon completion of 12 monthsVaccine Mandate PolicyIn accordance with federal mandates, all new employees, interns, and volunteers within Vinfen's Massachusetts programs will be required to be fully vaccinated (14 days beyond last shot of Pfizer or Moderna or 14 days beyond receiving a Johnson and Johnson vaccine), or have received the first shot in a two-dose vaccine (Pfizer or Moderna) with an appointment scheduled for the second dose. A 2nd dose must be received within six weeks of hire and the first shot prior to New Employee Orientation or have an approved exemption for medical or religious reasons by the first day of employment with VinfenWe are seeking a new PACT Nurse for our Program for Assertive Treatment (PACT) teams to provide treatment to adults with persistent mental illness. The PACT Nurse will work under the supervision of the Lead Nurse to conduct psychiatric assessments in addition to physical health needs in order to provide holistic nursing services. The PACT Nurse will be responsible for making appropriate referrals to community physicians, providing management and administration of medication in addition to providing a range of treatment, rehabilitation, and support services.Other responsibilities include but are not limited to:Provide case management for an assigned group of persons served, including coordinating and monitoring the activities of the individual treatment teamConduct a comprehensive assessment of psychiatric history (e.g., onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical healthProvide ongoing assessment, problem solving, side-by-side services, skill training, supervision, and environmental adaptations to assist persons served with activities of daily living.Response to Covid-19Vinfen continues to follow the guidelines determined by the Centers for Disease Control and Prevention, the Massachusetts Department of Public Health, and the Connecticut Department of Public Health.
