Under the direction of the Registered Nurse or Physician,
participates/assists in the nursing process during delivery of
patient care according to department policies and procedures.
Performs a variety of clerical duties, including but not limited
to, entering data into the Wound Care and Hospital databases,
answer telephone, greet customers, route calls to appropriate
staff, schedule patient and confirm patient appointments. Audits
data and generates reports as needed. Assists with wound
photography and measurements. Promptly reports unusual observations
or changes in patient condition/behavior. Completes all aspects of
patient care, including but not limited to, age appropriate
techniques for ADL's, mobility, transfers, positioning, hygiene,
toileting, and correct application of orthosis/prosthesis, and use
of adaptive devices. Transports patients. Collects specimens as
directed. Records complete, accurate and comprehensive data
pursuant to care given as well as observed responses to treatment,
including patient's participation in wound care plan. Completes
unit tasks such as cleaning exam areas and instrument processing.
Processes referrals and verifies patient insurance coverage.
Identifies the need for additional visits before the current time
or number of visits expire. Sends clinical information and
photographs (as needed) to appropriate insurance company with a
request for an extension of visits. Documents authorization of
extension in appropriate location(s). Assists with coordinating
patient ancillary testing with other hospital departments.
Assembles new patient charts, maintains and files existing patient
charts, "spot checking" charts for data completeness and signatures
(paper and/or EMR). Assists in orientation of other personnel to
the unit. Performs other work related duties as assigned or
requested by immediate supervisor.Consistently provides service
excellence to all patients, family members, visitors, volunteers
and co-workers in a manner that reflects Cape Cod Hospital's
commitment to CARES: compassion, accountability, respect,
excellence and service. Ability to read, write and communicate in
English. High School Graduate or GED. One year recent experience
(with past 5 years) as a Certified Nursing Assistant (C.N.A.),
Certified Medical Assistant (CMA), or equivalent (LPN, EMT). Two
years recent experience (within the past 3 years) in a patient care
setting such as a medical office, clinic or hospital. Demonstrated
computer keyboarding skills.
