Reporting to the Director, Ambulatory Specialty Practices, with a dotted line to the BID Plymouth Hospital Director of Nutrition Services, the Registered Dietitian provides optimal nutritional care to an outpatient endocrinology practice by assessing nutritional needs and developing nutritional care plans using the nutrition care process.
Job Description:

Interviews, assesses and develops plan for the institutional needs of clinic patients, in collaboration with attending physicians. Counsels individuals, families, etc. on insulin administration, insulin pump settings and diabetes management through individual sessions and group classes. Manages insulin titration per physician orders. Maintains effective written and verbal communications with physicians' offices. Documents education and assessment of outpatients in the electronic medical record. Maintains nutrition clinic records and participates in QA activities. Utilizes and develops resources and educational materials for patient education. Develops and maintains BID-Plymouth Diabetes Education program clinical quality initiatives. Facilitates community education and outreach specific to nutrition and diabetes management. Other duties as reasonably assigned.
