Nurse Practitioner, Hospice & Palliative Care (HPNP)

Job Description:


  • The Hospice and Palliative Care nurse practitioner (HPNP) works in collaboration with the Hospice staff to provide comprehensive, quality hospice and palliative care services under the direction and supervision of the Hospice Medical Director.
  • The HPNP will be responsible for providing patient/family visits for palliative and hospice patients both new and established and coordinating their plan of care with other designated hospice staff, the program director and the medical director.
  • The HPNP will function as an integral member of the Hospice provider staff.
  • The HPNP will provide medical expertise on pain, symptom management, and the evaluation of terminal prognosis to patients, family members and staff
  • The HPNP will on an as needed basis fulfill the requirements of the "face-to-face encounter" for beneficiaries enrolled in Cranberry Hospice. Patient visits will include visits to patient's homes, facilities, and BIDP Hospital. The HPNP will be available for consultation, collaboration or referrals from BIDP.
  • The HPNP will ensure that the care being provided is based on established hospice and palliative care guidelines and standards
  • The HPNP will provide expertise and oversight of pharmacy formulary and medication expenditures
  • The HPNP will interact with BIDP hospitalists, patient's PCP and specialists, as necessary, regarding the care and treatment of patients who require a palliative care consultation
  • The HPNP may act as the attending physician for identified Cranberry Hospice patients
  • The HPNP will regularly contact and collaborate with other health care providers regarding new patient referrals
    • The HPNP will supports the philosophy and mission of BIDP and Cranberry Hospice & Palliative Care in providing high quality hospice and palliative care to patients and families
    • The HPNP will provide leadership, support and development of BIPD and CH&PC staff
    • The HPNP will provide and develop collaborative relationships with other community agencies, building visibility of CH&PC in the community
    • The HPNP will participates in the design and implementation of programs designed to increase patient/family and referral source satisfaction.Education/Experience Required:
      • MSN from an accredited nursing school
      • Licensure in MA as a Registered Nurse and Nurse Practitioner in good standing
      • 2-4 years of experience in a medical specialty involved in the care and treatment of patients diagnosed with serious, advanced or terminal illness
      • A minimum of 2 years of clinical experience in oncology, pain and symptom management, palliative care and hospice, healthcare in a hospital or physician practice setting
      • Current American Heart Association BLS
      • Ability to obtain hospital privileges at BIDP
      • Valid driver's license and personal vehicle for on-the job transportation and evidence of insurance coverage as required by policyRegistration/Certification:
        • Certification by the American Academy of Nurse Practitioners and/or the National Board for Certification of Hospice and Palliative Nurses (NBCHPN-Certification for Hospice and Palliative Advance Practice Nurses) is preferred.
        • If not certified by NBCHPN, certification will be expected to be completed within a time period agreed upon between Hospice administration and the employee
          Knowledge and Abilities:
          • Comprehensive knowledge of hospice and palliative care practice
          • Excellent written, oral and presentation skills
          • Demonstrated leadership and customer service approach in hospice or related setting
          • Working knowledge of hospice EMR, Microsoft Office, including Word and Excel
          • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies
          • Knowledge of related accreditation and regulatory requirements
          • Knowledge of current principle, methods and procedures for delivery of medical evaluation, diagnosis and treatment in hospice and palliative care
          • Ability to work with other health care providers in a variety of settings
          • Knowledge and experience in working with an interdisciplinary team
            Location: Beth Israel Deaconess Hospital-Plymouth - Hospice
            Schedule: Full time, Day shift, 40; day shift
